WHO CAN WE HELP?
Families of youth, ages 3-18, who are newly experiencing mental health challenges.
Families who are in need of help navigating, understanding, and accessing mental health services for their child(ren).
Families in need of expanding their support system.
Families with children on waitlists for mental health providers.
Families with children who have experienced their first mental health inpatient admission.
Families who live within Columbia, Montour, Snyder, and Union counties
*Insurance, or lack of insurance, is not a barrier to this service.

Support
Attend meetings, helps manage waitlists and referrals, attend appointments, partner to develop and implement goals/plans, listening ear.
Can provide virtual or in person support.
Connect and Refer
Connect families to community resources, educational programs, services, and health-related social needs like housing, food, and transportation.
Referrals made or coordinated as appropriate
Education & Guidance
Education about mental health diagnoses, resources, and services. Linking families to trainings and webinars to help further their knowledge and give insight on their child’s needs.

The Family Navigator Program was created as a response to access issues that are present in the NorthCentral HealthChoices contract, specifically in the child/adolescent service areas.

By providing our families with support in the beginning of their journey, they are more likely to be connected to social and clinical supports they need to successfully navigate the behavioral health system.
